IMPORTANT NOTICE: Defect in SA1-2 Statement of Account Forms

The Copyright Office has informed us that the SA1-2 (Short Form) Statement of Account forms that the Office mailed to cable operators on or about December 10, 2007 contain improperly encoded barcodes on Space B on the form’s front page. The Office is in the process of sending out replacement barcode labels to each cable operator that received an SA1-2 Form with the defective label.
If you recently received an SA1-2 Form, you should be on the lookout for the replacement barcode label. When you receive the replacement label, you should affix it to the SA1-2 Form directly over the top of the original barcode in Space B.
The filing deadline for the Statement of Account forms is Monday, March 3, 2008. If you do not receive replacement barcodes for your SA1-2 Forms in the next few weeks, please let us know and we can assist you in obtaining the proper labels.
Only the SA1-2 Form is affected by the barcode problem. If you received Form SA3 (Long Form), you do not need and will not receive a replacement barcode label.
